I think it has to be a research topic book, not instructions or tutorials. Say you want to get a doctorate in Cows Milk, you would write up a 600 page book on the benefits of cows milk, and the healthyness of it etc etc.
My uncle and aunt (both are a couple lol) both got phd in a medicine field. My aunt did it in cows milk as I said above lol, and wrote a friggen 400 page book, with 9pt font or something on it. My uncle, did it on a brain stimulant, or something, thats in the brain, and wrote a 1000 page book, which is 10x15 in size, with size 9pt font lol.
Now they have awesome jobs at Oxford University for some research, so I guess years of education pays off.
Also regarding the calling yourself doctor. If you do it in a medical field, then you will be recognized as Dr. <name here> forinstance. I am sure you can go around calling yourself doctor, but people will not address you with Dr. <name here>.
