Fill out the info with whatever and hit Send. It will not send anything as it takes you to another page with suggestions of the FAQ section that may answer your question. If you read them and decide they do not help, then you hit submit and it actually sends your request.
This would save a lot of the emails asking for their password to be reset or other things that are already in the FAQ section.
I would guess they just modified the original contactus form to go to a middle page that has the FAQ. Then when you click on the Submit your Question link it finishes the process. I'm not sure if a modification already exists that will do this (did you try a search?).

Yes I did try a search. Did not find anything similar but I think the idea is excellent as it helps stop some of the questions from being asked.
