my main header template has vanished! urgent help

template id 9732 is my header template

its not in my remplate edit via apmin cp

and phpmyadmin cant find it either ! when i try adding it there are errors such as

  #1062 - Duplicate entry 'header-1' for key 2

i cant work it out its jsut gone !
