Erwins Form Hack Modification

I am using the hack here www.designerbeat.com its on the navbar as "New Contest"

but i need the form to have a custom title defined in the form. So all contests posted dont have the same title. Users can define the title from the form, and when its posted it shows that title

Can you tell me how. or if its possible?

Also in the form, if the freelance radio button is check i want it to go to a certain forum, and if the other radio button is checked which is professional, i want the thread to be posted into the professional forum.

Please Help! Also How Do i get the first post to look like the first post on vbulletin.org where it has customized bold text.
