Is there a vB3 version of the Moderator Application form released yet?

I've made one myself but if there's one released then I won't release mine (which isn't even complete yet, and I might not release it.)

what this form is made for ?! first time i hear about this...

There is a form mod available. It's not made specifically for moderator application, but I was able to change around some of the options in it to make it a moderator application form.

If you have one specifically for moderator application, then that may be worth releasing.
