I apologise in advance if this has already been done, although I have been unable to find this hack here.

I would like to prevent people who "reply" to a thread from selecting a message icon and entering a subject title, but allow people who start "new" threads to choose an icon and subject title.

This hasnt been done because it's not a hack but a template modification..

Just edit the template "newreply" via Admin CP/Templates/Modify and delete the relevant code..
