My friend wants to know how to make a php powered form...

He sent me this :

Want a form with fields such as ...

Name:

Address 1:
Address 2:
Address 3:

Email Address:

Description of Item:

want name, addresses, post code, phone number and email fields to be single lines.

want description of item field to be a multiline text box.

want a submit button and a reset button

submit button will send all the information in the field to specified email address.

reset form will reset form (obviously)

He also said he would prefer if it were made secure somehow, but that is not strictly important if it is too difficult to manage...

Can you help?
