Default Organized Posts and Replies in a Form

I really think this could be a good hack that could be used by a lot of people including vb.org. I wanted to create it, but it is taking me forever and I don't know enough about all of this yet to do it.

I would like to have one of my boards set up so that everyone that starts a thread has to include specific information. I will use vb.org as an example to what I mean. Someone has created a new hack and wants to post in the full releases. They click new thread and instead of the typical open box to type in the info about it they have a specific form similar to when someone first registers. It would be something similar to this.

Hack Name: |============| (<-------that's my symbol for a blank box that the user has to fill in )

Purpose of this Hack: |============|

Files to Modify: |============|

etc.........

I hope I made it fairly clear. Obviously I want mine to say different things then this, but I want it set up so that they have various things to fill in when creating a new thread. But I have thought of an additional idea that made this more challenging for me to create. I would like it so that the thread starter has 2 types of replies they can make. They can make a standard reply, or they can make a reply that again has a format to fill in that is an update to everything since the initial first post. It will have the same kinds of options but it will also give the user a chance to tell the updates to the thread, or hack in this case.

Additional ideas, since a query will probably have to be run to create new places for the data to be stored, I think it would also be great if the posts that this person made had some kind of look to it to make it easier to read. Such as maybe it alternates colors between the different answers. But that is just adjusting the template I guess.
