Added missing sections

This is just an expansion of a solution Firefly gave here

This hack (mini hack?) will allow you to set forums as requiring moderation for thread starting, leaving replies un moderated (or bice versa for that matter).

It adds a new setting to the forum table, "moderaterep", and requires editing of admin/forum.php, and root/newreply.php

I've also attempted to make a teeny weensy installer to make it a little easier to install. I know it's not brilliant, but it'll handle the alter table section for you.

Demo isn't available. If there's no bugs, I'll call this a full release.

Updated the installer to include additions to admin/forum.php that I missed putting into the installer.

If you've already installed this, the extra changes you want are the ones that update the database queries for add and doupdate
