Register Moderation NOT running

When registration is activated there is no problem, user get registered and they get the welcome message with no problem.

But when i activate the option that users just can get in when a moderator check the registration, the problem is:

-Users don't get the confirmation email.

-Users don't need to get aproved by a moderator, after 5 minuts they get auto aproved.
