We're getting members complaining that it takes forever for a post to show, and while processing, an error (system time out, I'm guessing) comes up. (A custom error message, saying that we're "updating" appears. No system email to admin is generated.) Often, we wind up with duplicate posts as a result.

I had it happen again this morning, repeatedly, but had no problem while simultaneously at the ACP.

What should I be looking for to determine the cause? This has been ongoing for months, but some days it gets crazy.

check your php version and also ask your host to check the cause from error logs.
