Miscellaneous Hacks - [Scandal's] vBContact Us

Description:
This modification allows you to add a new fully modulated "Contact Us" form to your vBulletin site. You can add/edit/remove sections and fields. It provides six (6) field types:

1. Single line text box
2. Multiple line text box
3. Single selection radio buttons
4. Single selection menu (drop down)
5. Multiple selection menu
6. Multiple selection checkboxes

Features:

• Fully modulated Contact Us form
• Six form field types
• Fields' section feature
• Fields can be "required"
• Two contact us e-mails allowed (= e-mails which receive the form)
• You receive to your e-mail the contact form with all its custom fields
• Human verification to prevent SPAM
• Contact Us to Thread feature (a discussion thread can be created for each form after its submit)
• Admin control panel form manager

Installation:
1. Upload all files of upload folder, to the correct directories.
2. Import *xml product.
3a. Add the link yourforum.com/contactus.php where you want OR replace the old contact us form.
3b. To replace the old contact form, go to admincp > (sidebar) Settings > Options >Site Name / Url / Contact Details and set the Contact Us Link to ?contactus.php? (replace the old ?sendmessage.php?).
4. Add one e-mail on "vBContact Us by Scandal - Options"

For example this version AND the original vB version, with one being a link (URL)?

The vBcontactus tab does not show for me. I installed EVERYTHING correctly with no errors. I even reinstalled. What can I do? It seems like a very useful mod.
