A Shoutbox for vBulletin CMS. Widget, guests, captcha...

Hello,
I can't find a shoutbox add-on (or kind of chat system) for VB4 that meets all my expectations, so I'm posting here to see if some people want to share some hints or ideas with me.

Here is what I need (minimum requirement):

- A simple shoutbox. I do not need privates rooms, just need a place where all people can shout simple text (well, smiley and text format could be appreciated but not required)
- "Secure" vBulletin user integration . I need that users loged-in in forum/CMS be recognized (nick names). And basic permission based on users' group on the forum.
- Moderation system. I need that messages can be deleted or edited.
- Allow guest users to use the shoutbox : read and write. This is the big issue, I need a minimum anti-spam, like captcha.
- A ban system. Based on VB permissions system (so, matching banned user group, or an other group)
- Can be added in the CMS, as a widget . Shoutbox widget or php/html widget.
- I do not want a external system. I need to host all of the content
- I do not want a Java applet or flash. I need the feature to be printed in HTML/CSS/JS/AJAX...

I can't find a good solution that offers all of these options...

What shout/chat system are you using for your web site (based on VB4) ? If, in your opinion there is no solution on VB.org (mod) what kind of tradeoff should I make ?
Is it a hard work / is it a good option to code a solution myself, considering I got some skills programming languages but I don't know much about how vBulletin works.

EDIT : ATM I've been using VSA Chat box for a couple of years and vBShout for testing purpose... both are cool but do not meet all my requirements...

We use a Tinychat video chatroom on our site but it doesnt offer a shoutbox(which we dont want)
