Navbar application tab and page

I am looking to add a tab to my forum that will allow a registered user to click on it, and then be taken to a page, that has my same theme, and fill out an application to be considered for full membership.
I would like to have this form present 5-10 questions to the applicant and when finished, that person hits submit and it would email out to the admin.

I don't know where to begin with this. All help is appreciated.

The scope of this may be easier than I thought.

With a little php tweaking I think what I need is already available.

I have created a navbar tab called application. I have linked the sendmessage.php file to the URL in that tab.

Here is where the tweak occurs. I want to remove the radio button choices and put in a paragraph of text.

The message field can stay intact.

Do you still think this should go over to paid request or can someone give me a nudge in the right direction on how to edit that php page myself?
