I want a plugin for my forums so people can click a link and they fill out a form.
I can set the questions, and once they have done, only people to a certain forums can check out the completed forms, such as an application for administrator, only the person that filled out the form can see it, and admins that have access to the forum where the forms are posted after they are done.

I don't know, but it looks damn silly. Maybe they just pay a lot for moderator positions? I can't imagine a decent volunteer going through a grueling interview like that.

If you are also paying that much for help then you should be able to pay coders to write a mod like this.

There are two mods that can do that:

1) Easy Forms

The asterisk disclaimer in your screenshot is a standard phrase in Easy Forms, so they probably used the first one.
