How to insert ads between forum categories and also between threads?

Where would I go and what code would I want to use to insert banner ads between my forum categories on my forum home page?

Same question, but I'd also like to insert banner ads between every 5 threads in each forum index.

Thanks!

Just in case my site is www.obesitydiscussion.com
