Ban / Infraction reason for posts

Hello, I tried searching for a mod or something that would give me this effect, but I didn't find anything. Basically, a mod to post the infractions by post like this would be very helpful. I've seen this used on multiple sites also, but I don't know how I could do this.



Currently, the admin has to hand type the post reason. Anyone know a mod or have / want to type some code for this?
