Im looking for a mod to allow me to create a form to ask questions like this one

I also installed

MGC Chatbox but the images are pointing to the creator of the theme www.massiveclan.com how do I change this?

Any help would be appreciated

I have uploaded the new style file with correct image paths.
Should work better for you now
