That is not done by a mod. You can do that by enabling the Block Manager at your Acp->Settings->Options->Forum Sidebar and Block Options. Then go to Forums & Moderators->Forum Blocks Manager and add the blocks from there. Hope it helps.

No html code is needed for what you are asking. Simply choose for Forum Threads when creating a new block to have the same block as in the attachment tha you posted
