In one of our subforums, I would like members to only be able to create new threads via a thread-creation form (created with this mod). I would like to replace the usual "Post New Thread" button in that subforum with a different button that links to the form in question.

Are there instructions anywhere for doing something like this?

I've never seen instructions. Just put a condition around it. If forumid==x, then show this button, else show default.
