Login fails 4.0.2 patch4

Hello I am using vbulletin 4.0.2 patch level 4.when login user ,admin.., after type login pass stil again ask login & pass why?but after login refresh then ok . Can you please help me fix this probleme!! My forum is .com/forum thanks

i am already finished this step manytime install/upgrade_402_salt.php still probleme !!!

Your board is closed now anyway, but really if you want someone to test it for you then you'll need to supply a testuser login (asking us to register is a bit much).

That's a user login, not admin, but that's okay if it's a live board. But it worked fine for me in Firefox.
