This is an example of how my forum is showing up. Can anyone tell me how to correct it?

It looks like you changed the order of the heading. Read this thread and it will tell you what template to modify to get it how you want - How To Change The Ordering Of The 'Last Post', 'Threads', And 'Posts' columns
