Form Generator

Hi all,
Just checking; anyone knows of any good form generator or program other than phpFormGenerator? The phpFormGenerator do not have the image verification (CAPTCHA) and can't control the filesize of the attachment and I need one that have these features.

Any good recommendation or advice?
