How to add [checkbox] I agree with the terms

I would like an upload form similar to the download update page for vBulletin. The page I am referring to is Download Security Fix for vBulletin 3.7.2.

I would like a "[checkbox] I agree with the terms of the 3DMyPage Terms of Use stated above" and a Scrol text box for Terms.

Can vBulletin do this and if so how?
