Strange attachment issue

Wierd problem here. When I post and add an attachment, it shows up in the posting body. Sometimes it shows up twice, once in the body, then again under 'Attachments'. For the life of me, I can't figure this one out, lol. Most everyone else's shows up under "Attached Images". (On rare accasion I'll find someone elses that does not) Any clues?

If you attach an image and then include it in your post using the [attach] BBCode (click the paperclip icon) it will not show up at the end of the post.

If you don't use the [attach] BBCode then they are listed at the bottom.

If you use [img] code and point it to the attachment it shows up in the post and at the bottom.

Ok, gotcha! But on occasion, when I use the [attach] code, it shows up in the posting body and at the bottom. only on rare occasion does it do this. Usually if I refresh the page or go back to it. Maybe a cache' problem on my pc?

Hmm I don't know, that post was just based on a few tests I did here just now.
