Paid Subsciption disclaimer

Hi,

I need to add an option to the paid subscription template. Before people can click on "Order using Paypal", they must read the disclaimer and check a box to certify that they understood it.

How can i add that ? Where do i put the code to verify that the box is checked or not ?
