I want a form on my forums like the one on this myBB board:

I did a search of MODs but nothing came up, but I thought I might've missed something so I'm asking here.

can't see anything there so enable guest access or supply a test account

Yeah, a screen cap would definitely help. Just by the title, I would have directed you to the Form Hack ("form", "All Mods", "Titles Only").
