I'm trying to create a new feedback forum and am having some trouble. The forum is added, but the formatting is not correct (see attached screenshot).

When I add these forums to my original main forum, they are added without issue. I would really like to have two separate forums (one for discussion, one for feedback).

Any ideas on this issue?

Looks like a template issue, i take it you forumdisplay template is modified, this could also be related to a hack.

If your forumdisplay template is default, disable your hacks and refresh to see if it goes away.
