Strange Problem

I just have a new user that went through the registration process and something happen, I received this email:

AND I CAN'T REMOVE IT FROM cP!!!
I had tried everything I know, still I can't remove it, its profile doesn't contain data....

What can I do?

Does this still happen if you disable the plugin system?
