How do atachments works?

Hi there.

I want to make a little customization, so I need some info about the process of atachments in the editor.

Wich files - variables are involved?

the editor toolbar can be modified by editing this template:

Admin CP -> Styles & Templates -> Style Manager -> ? ? -> Editor Templates -> editor_toolbar_on

but then this buttons call a function, where are this functions defined? I want to atach my own image, an image created by a php script, something like this:
