Adding captions to photos on upload

Has there been any mods or hacks out there that will allow a user to add a caption to their uploaded images? I have seen a few websites doing it.

Screen Caps below as an example.

	captiontext.jpg (34.0 KB, 0 views)
	captionfinal.jpg (26.0 KB, 0 views)
