Administrative and Maintenance Tools - Automatically Create Usernote on Usergroup Change

This hack is very similar to the hacks 'Automatically Create Usernote on Username Change' and 'Automatically Create Usernote on Email Change'.
This one creates a usernote in the user's profile when you edit the user's usergroup from the admincp.
It can detect changes in the primary usergroup, and additions+removals in the secondary usergroup.
