User Note Mod Request

Can anyone create a Mod that will automaticly create a user note when a profile field is changed. I saw a mod to do this for other things but not a profile field.

I require a name and mailing address when someone registered. I would like to see that whenever a member changes that info it creates a user note so the admins can keep track of addresses that a member uses. this would prevent problems with the kind of site I run.

Also if possible the ability to do multiple profile fields such as an email address field.
