Need help adding checkbox

I want to add a checkbox to manage attachments box right above the attachment key.
I need it to set field in db to 1 if checked when uploading the attachment.
I looked around in here but couldnt really find anything that would work in this spot.

thanx in advance to the kind soul who can help me out on this.
