Couldn't find something like this. But what I am looking for is a mod, where users (depending on usergroup permission) can add addresses and other members (depending on usergroup permissions) can view and even leave comments on the entry.

Basically I want to be able to add fields as necessary for the contact info and set the fields to required or optional. Even have it possible to split up the entries into different categories. The entry will show the user that added as well as date and time added, and will allow other users to comment on the entries.

Does anyone know if a mod like the exists? I have been searching for a couple of days using all the keywords I could think of and cannot find anything.
