For some reason one of our Forum Moderators was changed to a Super Moderator. None of the Admins or Mods made the change and it did not show up in the the Control Panel log. Anyone know what could have happened?

You can change the database using a query so that they'll be in the super moderator group.
This can be done in the admincp under maintenance or with phpmyadmin |

Is there a way to know if this was how this was done and who did it?

In your control panel log look for anyone using query.php
As for phpmyadmin, im not sure |
