Default Problem with banner not showing up on certain pages

We have a simple banner rotation script running, but the banner doesn't show up on certain pages? It shows up on the home page and when you click today's posts, but not on the general forum link?

Here is the script we are running on the header

PHP Code:
  <SCRIPT language=JavaScript>
/*
+---------------------------------------------------------+
Ad Simple                 Copyright www.YugDesign.com
+---------------------------------------------------------+
This program may be used and hosted free of charge by
anyone for personal purpose as long as this copyright
notice remains intact.
+---------------------------------------------------------+
*/

var img_width = "468";
var img_height = "60";
var img_title = "Click Here";

var ad=new Array()
//insert here your images src
ad[0]='http://www.mysite.com/forum/banners/banner1.gif';
ad[1]='http://www.mysite.com/forum/banners/banner2.gif';
ad[2]='http://www.mysite.com/forum/banners/banner3.gif';
ad[3]='http://www.mysite.com/forum/banners/banner4.gif';
ad[4]='http://www.mysite.com/forum/banners/banner5.gif';
ad[5]='http://www.mysite.com/forum/banners/banner6.gif';

var links=new Array()
//insert here your links
links[0]='http://www.link.com';
links[1]='http://www.link.com';
links[2]='http://www.link.com';
links[3]='http://www.link.com';
links[4]='http://www.link.com';
links[5]='http://link.net';

var xy=Math.floor(Math.random()*ad.length);
document.write('<td align=center><a href'+'="'+links[xy]+'" target="_blank"><img border=0<img src='+'"'+ad[xy]+'" width="'+img_width+'" height="'+img_height+'" alt="'+img_title+'"></a></td>');
</SCRIPT>
