I run a pretty successful healthcare site for physiotherapists, http://www.physiochat.com
I am trying to get my users to better present their case studies/patient profiles when adding a new post to ask for help. As a physio we always take a standard history, things like

Age and Gender of the patient
Presenting Problem/Condition
History of Presenting condition
24hour behaviour
Past or other relevent medical history
Other examinations and investigations (Neural Tests, X-ray, MRI etc)
Examination (active and passive testing)

I would like a new button next to the "new thread" button that says "New case presentation" that automatically creates the above headings in bold, with a few line break between them in the field that the user posts their message. That way they are prompted to give us a bit more information.

Maybe this could be a bit of BB code in the WYSIWYG but it would be neater as a new button before that. Could anyone assist in creating this?
