How do i add "Additional Required Information (profile)" ?

Hello,

How do i add more "Additional Required Information (profile)" in the "Registration Page" ?
I have added the "Date of Birth", but i forgot how to do it ?

And why i can not modify the "Discussion Title" that i created ? i can only modify the subtitle

Please help, thanks.
