I want to give people the option to add under their Avatar a list of three options:

A, B,C, D, E, F
and for each one selected, they can add a number from 1 to 1000

Of course a user could opt to have none, the first, 1st and 2nd, or all 3 options under.

The choices are the same, but a user should be able to add A 100 B 250 and E 150 as possible valid answers.

Any suggestions on setting this up to make it easiest for the users and cleanest on display?
