Help creating a form

Could somebody at least point me in the right direction...

I would like to create a form with 4 input boxes and a radial button next each box used to select between two different color fonts.

Then when the form is submitted I would like the entries viewable under the navbar.

I think I can handle the template edits to display the entries but I have no idea were to begin with the form....thanks for any help
