Need some help getting started....

I'm trying to make a page similar to these:

http://www.raceapeel.com/xcart/custo...del_signup.php
http://www.carandmodel.com/jobs/form_model.php

But I have no idea where to start. Obviously a page like this makes some kind of entry in the database right? I would also like it to, when filled out and submitted, be emailed directly to me.

If someone can make something like this, either in a product, or a few .php files or whatever need be, or point me in the right direction of how to make something like this, I'd appreciate it.

BTW: I WAS going to use the Forms hack but I don't like how you have to have the /newthread.php?do=model_app in there to make it come up.
