Where can I find a clan/guild application like this?

I want something like this for my vbulletin forum since my site is a LOTRO gaming clan. something i could easily customise to fit my needs.

Any ideas? Can anyone point me to a thread? I tried searching for "application" on here all I found was a "moderator" application.
