Automatic Post User Note on...

I am looking for a way to automatically create a new user note when a user:

Requests to join additional usergroups (including the note they put in when requesting)
When the requests are approved, and who approved them

Thanks in advance.

Ok...I wrote the first one...lol.

The second one is a bit more challenging...
