do there any form can let user leave message like this?

i hope my web can offer a service that user can leave some data
it is like http://www.boss33.com/goods/goods_de...p?gds_id=14943

i hope the service is combined within vbb
so that,user must log in the vbb and leave the message later.

or do you know any mod can do the effect?

i hope you can help me solve it,
if you do not know what do i mean completely,
please tell me and i will illustrate it more.

what i want to say is that

1.as the picture show



i can create many rows let user to leave data

2.after the user leave their info in step "1."
the data wre showed in the web
as the picture show



the mod i wanted
i hope it can let poeple leave some data public
and let others to read it
