The admin tools dropdown is probably just a bug Stefan missed. As far as the attachments scenario I didn't quite understand. Can you run through the exact problem your having from start to finish please ?

I uploaded a few images to a previous hack. (hack A)
I uploaded a few images to a new hack. (hack B)
One of the images in hack B is the same as hack A. Thus, I just reuploaded the same image.
The system (is working as it should per your settings) does not allow me to upload duplicate images.
I don't want to link to the image 'cause it will either show as a link or will display the original image (size) while the other images will display as thumbnail.
If the system was setup so that you can reuse images (already uploaded) problem would be solved.
