You really need to read the function and look at examples in the code. From cron/promotion.php:

            $userrank =& fetch_rank($user);
            if ($promotion['rank'] != $userrank)
            {
                $userupdates["$promotion[userid]"]['rank'] = $userrank;
            } 


$userrank is used directly.
