While I don't have the time to do a fix for you, I do see two problems...

1) You're using tables. vB4 is CSS driven.
2) You have a form nested in another form which doesn't work. (one of those scripts is a form)

You need to put the code in a <div class='???'> and define the class in CSS. And you need to move it outside of the form that's already on the page.
