if I go to admimcp -> Add New User that comes up on the top of the page... can anyone tell me what that is?

Usually I think, its wise to disable mod by mod, and each time test to see if you can replicate same issue, and just work thru until you find the problem. From memory, it usually implies a duplicate field entry that is conflicting within the database, but I am no expert.. lol
