surely the under 18 thing would be doable with a simple line of code change... just change the under 13 thing to under 18 instead. Stop the coppa users from seeing anything and your issue is solved.
Another suggestion, we have a disclaimer form which members have to fill in to access our adult forum; you could do this on registration with a custom profile field. Having someone type in 'I am over 18' and that they agree to the fact that the site is adult oriented and they are legally able to access that material in the country they reside in would help greatly
The first one is a template mod, with a custom profile field.
